electronic health records system and we are asking for your help. The system
includes a patient portal which will allow you to request appointments on
line and to receive laboratory work and other important information
electronically in a secure way. We are asking you to help us make the
transition as smoothl as possible, You now have the ability to register for
the portal and start the process. Once you have registered, you will be able
to go on to the system and enter much of your past medical history which we
will be able to review and place directly into your electronic file. We
assure you that this is all done in a secure environment. Please help us to

the timely manner in which you have become accustomed. By having your
information already avilable to us in an electronic format, it will help
make your appointments go more smoothly and it will allow us to focus on the
problems at hand when you come in to the office with a new medical problem.
